How to Refer

  • At this time, referrals must be initiated by the school, based on defined eligibility criteria
  • Schools must obtain consent from parent(s)/guardian(s) for referrals and sharing of school  documents relevant to these referrals (e.g. IEP, report cards, safety plans or other professional reports)
  • Referrals may be faxed, mailed or uploaded electronically using ServU, a secure document transfer platform that is compliant with all Canadian privacy legislation and ensures that only the QCTC can view document(s) uploaded; instructions are included below

Submitting a referral electronically using ServU:

1.     More than one document can be uploaded, as needed. Please be sure to name
and save your document before uploading; if a blank document is received,
there is no mechanism to determine where or who it was sent from.
 
2.     Click here and select “Browse”.
 
3.     Select your document from the location where you have saved it and click “Open”
at the bottom right side of your screen.
 
4.     Once the file name appears in the box to the left of the “Browse” button, navigate
to the bottom right corner of your screen and select “Upload”.
 
When a document is uploaded, a screen will appear confirming the upload; however, the
system does not generate a reference or confirmation number. Please feel free to contact
us to confirm that we have received your document(s).
