To refer to SBRS Occupational Therapy and/or Physiotherapy. Please complete the following:
- Referral Request
- Consent for Referral and Information Sharing
- Additional Information for Therapy Referral (OT/PT)
*Please note that referrals for Occupational Therapy must be reviewed with the SBRS OT
assigned to the school prior to submitting.
